Athletic Clearance
Required for all incoming athletes. Due JANUARY 15 to TP Athletic Office in the Learning Commons.
– All 2023 athletes must complete
1) the online clearance (athleticclearance.com) and turn in a signed confirmation page
2) physical exam forms
3) copies of the front & back of the insurance card, and
4) the private auto use form. You will receive an email once you are cleared to participate.
– ALL Athletes who participate in a previous sport (fall or winter) must still complete the online clearance (athleticclearance.com) and turn in a signed confirmation page.
