Athletic Clearance
Required for all incoming athletes. Due JANUARY 15.
– All athletes must complete
1) the online clearance (https://www.homecampus.com/login)
2) upload the following documents:
-physical exam form
-front & back of the insurance card, and
-the private auto use form.
You will receive an email once you are cleared to participate.
Athletes who participate in a previous sport (fall or winter) must add track & field to their athletic clearance
Transportation Release Form
- Parents, if you would like to take your child to and from a track meet, please complete the Transportation Release Form and email this to tpathletics@sduhsd.net. Please also include Coach Charlenne (charlenne.falcisstevens@sduhsd.net) and Coach Ryan (ryan.bath@sduhsd.net).
